SOCIAL WORKER CASE MANAGEMENT RFT

Job summary: The Utilization Management/Case Management Social Worker is a Licensed Professional Social Worker who functions within accepted boundaries of social work practice as defined by the New Mexico Board of Social Workers, and The New Mexico State Licensing requirements for Social Workers and Gila Regional Medical Center (GRMC) policies and procedures. She/he is responsible for working with the Utilization Management/Case Management team to provide social work services and collaborates with the medical and nursing staff to assure that patients are receiving said services. She/he is also responsible for recommending and/or facilitating the discharge planning process for assigned patients with medical, nursing, and other department staff as well as patients with their families and outside agencies to assure that patient specific services taking into account unique factors about that patient and their circumstances are available at the next level of care when the patient is discharged from GRMC. She/he completes appropriate documentation both internal and external necessary to document concurrent review and discharge planning activities. She/he takes into consideration the age of the patient at all times in assess, planning and implementing patient care. This job is classified as Category III relative to infection control risk.
Qualifications required:

MUST BE LICENSED in social work in the State of New Mexico; either LBSW or LISW**

CPR preferred

Minimum of 1-year experience in either long-term care, community based program or acute care setting

Computer literate in both GRMC and Windows applications

Bilingual preferred (Spanish/English)
Other comments:

Utilization Management/Case Management services available 5 day per week during normal business hours (8:00 am to 4:30 pm)

Must be flexible in hours to assure communication with GRMC staff, family conferences and physician rounding
