The Hospice Clinical Coordinator (HCC) is a Registered Nurse who is responsible for assessing, reassessing, planning, directing, evaluating, scheduling and delivering patient care to the patients in the Hospice Department.  The HCC is responsible to assure provisions of safe, prudent, and economical patient care according to His/Her thorough knowledge of hospital, departmental policies and procedures, GRMC standards of Nursing Care, Standards of Nursing Practice and his/her own skills/knowledge as an RN.  He/She will perform all duties in compliance with approved standards of care and practice as defined by GRMC, the New Mexico Nurse Practice Act and the Joint Commission for Accreditation of Healthcare Organizations (JCAHO)

• Registered Nurse with current New Mexico License.  BSN preferred
• CPR Certification
• Demonstrates knowledge of Home Health/Hospice care and/or Public Health knowledge
• Demonstrates clinical competency with skills necessary for home health/hospice nursing
•  Leadership/Supervisory skills preferred.
•  Minimum of 1-year progressive experience in home health/hospice/public health.
• Has a thorough knowledge of English, including spelling syntax and punctuation. 
• Must have or be willing to obtain OMI Certification within 6 months.

• Normally Monday through Friday 8:00a – 5:00 p.  Work requirements may also occur before or after usual working hours.  Administrative On-Call after office hours.
