Center’s Case Management
Department consists of four nurses and one social worker providing discharge
planning, utilization management and social services.
Their role is to coordinate and facilitate
access to timely and appropriate health care services, and to ensure continuity
of care through the recovery process.
Case Management is available to any patient, family member,
or caregiver. The department is often
called upon to assist with social or emotional issues that may have an impact
on a patient’s medical condition, rate of recovery or discharge plan.
Case Management is adept at networking with a wide variety
of community programs, resources and institutions; and is familiar with options
to support both in-home and out-of-home care after a hospital discharge and how
each option is funded.
