What to Bring to Registration:

• Insurance identification cards
• Social Security number
• Medicare or Medicaid card
• Current medication list
• A method of payment for any co-payments, deductibles or coinsurances that is due
• Information on next of kin or responsible parties including name, address, phone, and relationship
• If you come for outpatient services please bring your doctor's order with you or verify with your doctor's office that it was sent in
